Would you
like to join C.H.E.F. of Lafayette? Just download and

Please read and print out both pages. (You may
also be able to fill the form out in Acrobat reader by
clicking in the desired field and typing in your
information. You can then print your form already filled
out.)
Completely
fill out the form and mail both pages in to the
address below*, along with your membership dues. Dues
are $40.00 if you want to receive your newsletter via U.S.
Mail, or $35.00 if you prefer to receive your newsletter
via email. With your paid membership, you will receive a
newsletter, privileges to CHEF activities such as field
trips, a member directory, a discount on membership to HSLDA, a discount on sports session registrations, and a
discount to the annual CHEF of Louisiana Convention and
Book Fair. (Scroll down to see the benefits of
